Student Registration Form
Form that must be completed to register a new student in the DistrictParents of NEW students entering the District, (PS-12Parents should complete the form and present to Guidance Dept. upon registering a new student.
Free and Reduced Lunch
(new form to be posted) 
Application for your child to receive free or reduced priced school meals. You need to submit only one form per family, and list all children on the one form.1 form per familyRead entire application. Print the last two pages and complete Parts 1-7, staple the two pages and return to homeroom teacher or 1st period teacher.

Address ChangeNotifies school of any address change you may have had since last school year.Students that have had address changes.Complete the form and return to the building office, or homeroom teacher or 1st period teacher.
 
Explains acceptable use policies for using the Wheelersburg network and District computers. Until the signature page is returned, students will not be issued their computer network account.All students K-12Parents and students should read through entire policy, then print and sign the signature page at the end and return to homeroom teacher or 1st period teacher. Only the Signature Page needs to be returned.

Pesticide Notification Form

Parents may request to be notified in advance before any service visit conducted by a pesticide business.

 Open Enrollment Form 2017-18 Used to request that a student living outside the District be accepted to attend Wheelersburg Required for any student wishing to attend Wheelersburg but does not live within the District boundaries    Form to be completed by parent/guardian and returned to the Superintendent's office.

HS Handbook
 Signature Form

Signature form acknowledging recepit of the student handbook and that students and parents are aware of the code of conduct and punishment if rules in the Handbook are not followed. Required by Ohio Revised Code and the State Board of Education.High School StudentsMust be signed by both parent and student. Return to 1st period teacher.
 The Policy contains the board approved policiens and procedures for HS students chromebooks.
The Loan Agreement and Protection Plan forms must be signed  before a chromebook will be issued to a student.
 Parents and Students in grades 9-12, attending classes in the high school building. The policy should be read by both students and parents. The loan agreement and protection plan forms are to be signed and returned to Tech Office.
Athletics Insurance Waiver Form needed to be completed by all athletes.Parents and students in grades 9-12Must be signed by parents and returned to the school.
